About this template
An editable Excel register of the personal protective equipment you issue to workers, contractors and visitors. Record the worker, the item, the task or risk it controls, the marking on the item, the size, the date issued, the fit check and the training given. A replacement date you take from the manufacturer’s information turns amber and red as it comes due. There is a signature column for the printout and no column for charges. No law requires a PPE issue register: it helps you show you have met the duties that do exist.
What Is Included In Our PPE Issue Register?
- A Start Here sheet: type your details once and they appear on every sheet and printed page
- One row per item issued: worker, worker type, item, task or risk it controls, marking, size and quantity
- Date issued, who issued it, and whether it came from you, a labour hire company or a host employer
- Fit check date, training date and who gave the training, and a hearing test date for hearing protection
- A replace or check by date with an automatic status that turns amber and red
- Returned or replaced date, the reason, defect reports and a worker signature column for the printout
- A plain-English summary of the PPE duties of businesses and workers, with no charge to the worker
Legislation and Codes of Practice Referenced
- Work Health and Safety Regulation 2011 (Qld): ss 35 and 36 (eliminating risks, hierarchy of control measures), s 39 (information, training and instruction), ss 44 to 46 (PPE: provision and use, PPE used by other persons, duties of workers)
- Work Health and Safety Regulation 2011 (Qld): s 58 (audiometric testing), s 529B (respiratory protective equipment when processing crystalline silica)
- Work Health and Safety Act 2011 (Qld): s 273 (no levy or charge on workers)

Is it compliant with Queensland law?
It's written around Queensland's Work Health and Safety Act 2011 and Regulation 2011. Tailor it to your site, consult your workers and review it whenever conditions change; no template can guarantee compliance on its own.
What if it isn't right for my job?
If it doesn't match its description or has a fault, we'll fix it, and if we can't, we'll refund you (or give you store credit if you'd prefer). Check the sample pages first, as we don't refund a change of mind.
