About this template
An editable Excel register of the safe work method statements your subcontractors give you. Record the subcontractor, the SWMS and the high risk construction work it covers, when it was received and reviewed, and the outcome. A check column flags work that started before a SWMS was received, and a keep-until date follows the WHS Regulation rule for keeping a SWMS. The law does not require a SWMS register: it helps you show you have met the duties that do exist.
What Is Included In Our Subcontractor SWMS Register?
- A Start Here sheet: type your details once and they appear on every sheet and printed page
- One row for each SWMS: subcontractor, ABN, trade, title and revision
- The high risk construction work covered, chosen from the section 291 list
- Dates received and reviewed, who received and reviewed it, and the outcome
- A check that flags work started before a copy of the SWMS was received
- Review triggers and the new revision after a review
- Related permit and sign-on record references
- Incident and completion dates, with a keep-until date that follows the section 303 periods
- A plain-English summary of the Queensland rules on SWMS, principal contractors and keeping records
Legislation and Codes of Practice Referenced
- Work Health and Safety Regulation 2011 (Qld): s 291 (high risk construction work), ss 299 to 303 (safe work method statements), s 309 (WHS management plan) and s 312 (principal contractor to obtain SWMS)
- Work Health and Safety Regulation 2011 (Qld): s 38 as applied by s 302 (review of control measures); s 301 (copy to the principal contractor)

Is it compliant with Queensland law?
It's written around Queensland's Work Health and Safety Act 2011 and Regulation 2011. Tailor it to your site, consult your workers and review it whenever conditions change; no template can guarantee compliance on its own.
What if it isn't right for my job?
If it doesn't match its description or has a fault, we'll fix it, and if we can't, we'll refund you (or give you store credit if you'd prefer). Check the sample pages first, as we don't refund a change of mind.
