About this template
A free Excel register for workers signing on to a safe work method statement. Record the SWMS title and revision, the job or site, each worker’s name and business, the date, who briefed them and their signature on the printed copy. When a SWMS is revised, mark the workers who need to sign again and they stand out in yellow. The Start Here sheet explains plainly that the law does not require a sign-on register, and what it does require of a SWMS.
What Is Included In Our SWMS Sign-On Register?
- A Start Here sheet: type your business details once and they appear on every sheet and every printed page
- One row for each sign-on: worker name and business or employer
- SWMS title, SWMS revision and the job or site
- Date signed on, who briefed the worker and a signature column for the printed copy
- Sign-on status: signed on, re-sign needed after a revision, superseded by a newer revision, or work finished
- Workers who need to sign a revised SWMS highlighted in yellow and counted on the Start Here sheet
- Room for 300 sign-ons, with print-ready pages
- A plain-English summary of what the WHS Regulation says about preparing, following, reviewing, keeping and handing over a SWMS, with a clear note that a sign-on register is good practice and not a legal requirement
Legislation and Codes of Practice Referenced
- Work Health and Safety Regulation 2011 (Qld): ss 299 to 303 (safe work method statements: preparing, complying, copy to the principal contractor, review and keeping), s 291 (high risk construction work), ss 38 and 39 (review of control measures; information, training and instruction)
- Work Health and Safety Act 2011 (Qld): s 19(3)(f) (information, training, instruction and supervision)
