About this template
A free Excel register for the high risk work licences your workers hold, such as forklift, boom-type elevating work platform, dogging, rigging, scaffolding and crane licences. Choose the licence class from a list of the classes in Schedule 3, type the licence number, issuing state and expiry date, and record who saw the licence and when. Expired licences turn red, licences expiring in the next 60 days turn yellow, and the sheet works out how long to keep the evidence.
What Is Included In Our High Risk Work Licence Register?
- A Start Here sheet: type your business details once and they appear on every sheet and every printed page
- One row for each licence: worker name, position, licence class, licence number and issuing state
- A drop-down of every licence class in Schedule 3 of the WHS Regulation, using the exact class names
- Expiry date, with the days left worked out automatically
- Licence status: licence held, renewal applied for, training under supervision, certified and not yet licensed, suspended or cancelled, or worker has left
- Evidence sighted: the type of evidence, who saw it and the date
- A last-date-of-work column that works out how long to keep the evidence (at least 1 year)
- Expired licences in red and licences expiring in the next 60 days in yellow (a licence with a renewal applied for is left out), and licences with evidence not yet sighted flagged
- A plain-English summary of the Queensland high risk work licence and evidence rules
Legislation and Codes of Practice Referenced
- Work Health and Safety Regulation 2011 (Qld): s 81 and sch 3 (high risk work licences and classes), ss 82 to 84 (exceptions, licences from other jurisdictions and direct supervision)
- Work Health and Safety Regulation 2011 (Qld): s 85 (evidence of licence and record keeping), ss 92 to 94 and 101 to 103 (duration, licence document and renewal)

Is it compliant with Queensland law?
It's written around Queensland's Work Health and Safety Act 2011 and Regulation 2011. Tailor it to your site, consult your workers and review it whenever conditions change; no template can guarantee compliance on its own.
What if it isn't right for my job?
If it doesn't match its description or has a fault, we'll fix it, and if we can't, we'll refund you (or give you store credit if you'd prefer). Check the sample pages first, as we don't refund a change of mind.
