About this template
A one-job permit that records the steps the Queensland WHS Regulation expects before people work where they could fall. It says plainly that it is the business’s own permit system and that no law requires a work at height permit. It works with your SWMS and risk assessment.
What Is Included In Our Work at Height Permit?
- The task, place, fall height and roof slope
- Which fall rule applies, with the housing, 3 m, 2 m and 26 degree tests set out exactly
- Controls chosen in the order the Regulation sets
- Equipment and anchor checks, with scaffold, ladder and EWP pointers
- Rescue arrangements and the SWMS reference
- Crew briefing, authorisation and close-out

It's written around Queensland's Work Health and Safety Act 2011 and Regulation 2011. Tailor it to your site, consult your workers and review it whenever conditions change; no template can guarantee compliance on its own.
